When flying, diving, or climbing at high altitudes causes tooth pain, it’s not always a coincidence—it could be barodontalgia.

Barodontalgia is tooth pain triggered by changes in air pressure. It occurs when trapped air inside a tooth expands or contracts under low-pressure conditions—commonly during airplane takeoffs and landings, scuba diving, or high-altitude activities like mountaineering. These pressure changes affect the air inside the tooth and surrounding tissues, leading to discomfort.

Pain may range from a sudden, sharp stab to a dull, lingering ache. It can be localized to one tooth or radiate to other areas, and is often accompanied by tooth sensitivity.

Barodontalgia is often a sign of an existing dental problem, such as:

  • Pulpitis or periapical inflammation – Infection or inflammation of the tooth pulp (root) aggravated by pressure changes.
  • Faulty restorations – Poorly fitting fillings or crowns that trap air or fluids, sometimes even leading to tooth fractures.
  • Tooth decay (caries) – Cavities exposing sensitive inner layers that react painfully to pressure changes.
  • Sinus inflammation or infection – Can cause referred pain in the upper jaw, which worsens with pressure shifts.
  • Cracked tooth or fillings – Tiny cracks that become more painful with pressure variations.
  • Take over-the-counter painkillers such as ibuprofen or paracetamol.
  • Rinse your mouth with warm salt water.
  • Apply oral pain-relieving gel to the affected area.
  • Brush and floss regularly to maintain good oral hygiene.
  • For frequent flyers, divers, and mountaineers—stay consistent with dental care.
  • Schedule regular dental checkups and complete recommended treatments.
  • Avoid extremely hot or cold foods and drinks before activities involving pressure changes.
